1. Collect  the dust from the whole floor surface of each room of your house, sweeping up the floor by a broom.

2. Put the material so collected in separate  plastic envelopes  or in small  jars  (like the ones used for the urine), write on each container the name of the room in which you have picked up the sample of dust as you prefer (for example: living room, kitchen, single bedroom and so on)

    

    

3. Unload the “anamnesti card” and fill  in it carefully. Download

4. Made your payment in advance as follows and print a copy of your wire payment.

   

5. Put the dust sample and the anamnestic card and a copy of your payment in an envelope or in a box and send everything to the following address:
